  School forms are official documents used to track and record various aspects of a student's academic and personal information within a school systemThey are used by teachers, administrators, and other school personnel to manage student records, track attendance, and assess student progress. Some common examples include School Form 1 (SF1) for school register, SF2 for daily attendance, and SF10 for permanent academic records. 





Key School Forms and their purposes:

  • SF1 (School Register): A list of all students officially enrolled and attending classes. 
  • SF2 (Daily Attendance Report of Learner): Tracks student attendance on a daily basis. 
  • SF3 (Books Issued and Returned): Records books and other materials issued to students and their return. 
  • SF4 (Monthly Learner's Movement and Attendance): Summarizes learner movement (transfers, dropouts) and attendance for the month. 
  • SF5 (Report on Promotion and Level of Proficiency): Records a student's academic performance and results of assessments at the end of the school year. 
  • SF8 (Learner's Basic Health and Nutrition Report): Contains information on a student's health and nutritional status. 
  • SF10 (Learner's Permanent Academic Record): A comprehensive record of a student's academic history, including grades, courses, and other relevant information. 
  • SHSF-1 to SHSF-7, SF8-SHS (Senior High School Forms): Specifically designed for senior high school students, including versions of SF1-7 and SF8 tailored for the SHS curriculum.
